Welcome to the 2023- 2024 school year. Please see a list below of commonly used forms. If you have any questions or concerns, please don't hesitate to reach out. 

  • Allergy and Anaphylaxis School Care Plan & Orders 

    • Any medication (extra epi-pen to be kept in the nurse's office) should be sent in a labeled Ziploc bag with the student's name, DOB, and grade

  • Asthma School Care Plan & Orders

    • Any rescue inhaler or nebulizer supplies should be sent in labeled Ziploc bag with the student's name, DOB, and grade

  • Diabetes School Care Plan & Orders

    • Any supplies should be sent in a labeled Ziploc bag or Rubbermaid bin with the student's name, DOB, and grade

  • Seizure Action Plan

    • Any medications should be sent in a labeled Ziploc bag with the student's name, DOB, and grade

  • School Medication Administration Form - for any medication not provided by the school Excluding inhalers, EpiPens, diabetic or seizure medications (use the specific forms above)

  • "Annual Health Office Update Forms" are found in the parent portal in Genesis and must be updated and signed yearly which permits the school nurse to administer: Acetaminophen (Tylenol), Ibuprofen (Advil), Midol, Tums & cough drops.

Prescribed Medication:

As per the State Dept. of Education, any prescribed medication needs to be delivered to the nurse's office by a parent/guardian, in original pharmacy packaging with a label along with a physician's order and parent/guardian written permission (see form above).

Ask your pharmacist for the medication to be divided into two bottles completely labeled- one for home and one for school. Do not send medications in baggies, tissues, envelopes, etc.
